- Mailing
Address
- What
you are requesting (i.e. application, other information)

requesting an application, please indicate the year and
semester you will be applying for.

If you have
recently changed your name, address, or phone number, please
complete a change form available at the Office of Admissions
& Records. We can then make the appropriate changes to the
information we have stored in our student record files.
Students
are responsible for keeping the Office of Admissions & Records
(Queen Lili'uokalani Center for Student Services, Room 001)
informed of their correct mailing address. Your current mailing
address is where correspondence from the University will be
sent. Your permanent address is where your 1098-T tax form will
be sent in January of amounts billed in the previous calendar
year (excludes non-resident aliens).
NEW:
For your convenience, you may also change your mailing address
and phone number online via MyUH Portal. Connect to the UH Manoa
Registration Homepage www.hawaii.edu/myuh/manoa
to login to MyUH Portal "Academic Services" and select
"View/Update my Address(es) and Phone(s)".
The Student
Data Change Form is also available as an Adobe Acrobat PDF (Portable
Document Format) file. This form is for updating your current
mailing address, permanent address, and phone number only.
To view and print the document you will need to have the Adobe
Acrobat Reader installed on your computer. If you do not already
have the Acrobat Reader you may download
it from Adobe.
If you wish to submit a written request, please include the
following information with your signature (please type
or print legibly - illegible and unsigned requests cannot
be processed):
Full
Name
Social Security Number or UH Number
Student Level (i.e. Undergraduate, Graduate, Law, or Medicine)
Currently Enrolled at UHM? (If no, when were you last
enrolled?) |
Specify
which items are to be changed (mailing address, permanent address,
phone number).
Mail
or turn in your change of address form to the Office of Admissions
