2004 Forms Availability

Release Date: 2 December 2003

The 2004 IACUC Forms (Vertebrate Animal Use Protocol, Follow-up Annual Renewal, and Revision) and the respective form Instructions are now available on this website.  Applicants are encouraged to begin using the 2004 Forms immediately. 

Minor changes have been made to each form from the previous year.  Please be advised that all submissions to the IACUC must be made using the 2004 Forms effective January 8, 2004

The forms at the website are form-fillable.  Applicants may elect to request that the forms be forwarded to them electronically by contacting the Compliance Office via email at deniseye@hawaii.edu or nmagno@hawaii.edu.  Hardcopy versions of the forms may also be obtained at the Compliance Office at this address:

With the exception of holidays, applications are normally due on the second Wednesday of each month by 12:00 (noon).  The committee meets regularly on the third Thursday of each month.  Refer to the Meeting Schedule for the respective submission due dates and committee meeting dates.
