Faculty, Staff and Students...
Reservations being taken now.
Limited to 80 appts.
Free to HMSA members (must bring HMSA card & suscriber's birthdate & ID).
$40 exact change/check for Non-HMSA members (bring ID).
Faculty & Staff will have consent forms placed in their mailboxes to complete & bring clinic day.
Students are asked to stop by Health Office by Monday, Oct 19 & bring HMSA card & suscriber's birthdate to complete consent forms.
Reservations taken from 1pm to 2:15pm. Standbys are invited to check from 2pm to 2:45pm if any vaccines might become available (maximum 80 shots can be given out that day).
