NOTE that the Department of Curriculum Studies also requires all
applicants to submit to the department an Emphasis Area Form, an Express Application with Statement of Objectives Form. Applicants should arrange for three professional letters of reference to be sent directly to the Secretary at the Curriculum Studies office (see above right for the address). Pro formas for these items will be mailed by the department on request by phone or mail, or may be downloaded in one packet here.
