your name is on your response.  (Return this page in addition to

1.  Identify the academic and personal strengths that you feel are
    arguments for admission to the PhD program.

2.  Identify areas of academic and personal development that you
    would like to strengthen while pursuing the degree.

3.  Do you intend to be a full time student until you obtain the
    PhD?  If not, identify your other commitments.

4.  If you are admitted, how many years of study do you anticipate in
    completing degree requirements?

5.  Either attach a vita or briefly summarize your academic and career
    development to date.

6.  Discuss in detail your professional goals and how the program in
    Educational Psychology relates to your goals.

7.  Specify future directions your career might take within the
    discipline of educational psychology.

8.  Since the dissertation is a major facet of the doctoral program, a
    written sample of your ability to define a relevant problem in the
    domain of educational psychology and to outline an effective
    strategy for investigating that problem is required.  This
    requirement is most appropriately met by submitting a copy of your
    Master's thesis.  If you did not complete a thesis, submit a
    research paper completed as part of your Master's (or
    undergraduate) program.  If you have completed neither a thesis
    nor a research paper, attach a research proposal focussed on a
    problem of interest to you.  It is important that you include as
    part of the proposal and introduction to the problem, a statement
    of specific hypotheses to be tested, research method to be
    followed in collecting appropriate data, and a discussion of
    statistical procedures to be utilized in analyzing collected data.
