International Students Scholarships Check and Tax Forms Help Page
This information is provided to assist international students who received scholarships and need help filling out the check and tax forms.
Check Request Form for Student Aid Recipients (Pink form)
- Please fill out just the "Recipient Information" which is the information inside the box.
- In the information field marked “Address” provide your address in your home country.
- In the information field marked “SS#/ITIN#” enter your Social Security Number (SS#). If you do not have a SS#, provide your UH Student ID number in the box and go immediately to International Student Services and register for a SS#.
- In the information field marked “Phone #” provide a Hawai`i phone number.
- In the information field marked "Mailing address for tax form” provide the address where you want your tax forms mailed to.
- Don’t forget to sign and date the form!
Please note that 14% of the scholarship amount is withheld for taxes unless there is a tax treaty between the U.S. and your home country.
Tax forms are mailed to you at the beginning of the next year (usually in January). Some or all of the withholdings may be included in your tax return.
Foreign National Information Form (Green form)
- (8) “Visa #”: Enter the 8-digit number in red on your passport.
- (13) “What is the actual date you entered the United States?”: Enter the very first date that you entered the U.S.

- (14) “What is the start date of your immigration status for this primary activity?”: Enter the date that you entered the U.S. for the “primary activity” listed on your I-20 statement of purpose. This date will be the same as in (13) if this is your first trip to the U.S.

- (15) “What is the projected end date of your immigration status primary activity?”: Enter the date that you expect to be leaving the U.S. This is just an estimated date.

- (Page 2) “Please list any visa immigration activity in the last three calendar years and all F, J, M or Q visas since January 1, 1985”: List all travel in an out of the U.S. in the time specified. Especially list all F-1 visa activity. List the dates that you entered and exited the U.S. List J-1 subtypes if applicable. “Primary Activity” is the reason you entered the U.S. for that trip (e.g. “college”).

Please check to see if you qualify for a tax treaty with the U.S. (Note: Taiwan and Hong Kong are not part of the People's Republic of China and have no tax treaty with the U.S.).
Please sign, date, and provide a phone number and email on both the back and front of this form.
Certificate of Foreign Status of Beneficial Owner for United States Tax Withholding, Form W-8BEN (White form)
- 2. “Country of incorporation or organization”: Enter your home country on your passport.
- 3. “Type of beneficial owner”: Check “individual”.
- 4. “Permanent residence address”: Provide your home country address (if you've been in the U.S. longer than 5 years, provide your U.S. address).
- 5. “Mailing address”: Provide your local address.
- 6. “U.S. Taxpayer identification number”: Check the box for “SSN or ITIN”. Enter your Social Security Number (SS#). If you do not have a SS#, provide your UH Student ID # in the box and go immediately to International Student Services and register for a SS#.
- 8. “Reference number”: Leave blank.
- 9. “Claim of Tax Treaty Benefits”: If you qualify for a tax treaty (to find out, check the list on the back of the green form), fill this part out by checking box “a” and providing your home country in the blank. If you don't qualify for a tax treaty, leave this part blank.
- 10. “Special rates and conditions”: Leave blank.
- 11. “Notional Principal Contracts”: Leave blank.
Please sign and date the form. The “Capacity in which acting” information field should be filled out as “self”.
