Presidential Scholars Travel Grant request form (pdf file): This form must be filled out by all Presidential Scholars seeking a $2000 travel grant. Please attach a description of your travel plans and how it will enhance your education objectives. Return this form and the attached proposal to Dr. Lee Putnam, SSC 413, 2600 Campus Road, Honolulu, HI 96848 or via fax at (808) 956-9240.
