Biological Safety

PRINCIPAL INVESTIGATOR CHECKLIST FOR BIOLOGICAL COMMODITIES USE

Use this checklist to ensure the proper procedures have been followed before importing, transferring or moving biological commodities from you previous laboratory to the University of Hawaii. Biological commodities include: recombinant DNA, plants, animals, micro-organisms, biological toxins, human materials, cell cultures, etc.

APPROVAL AND PERMITS

[  ] Federal permits to transport or import plant pests or etiological agents, i.e., USDA or CDC.
[  ] Import and Use Permits from the Hawaii State Department of Agriculture (HDOA).
[  ] Federal or State permits for scientific use and collection, i.e., Fish and Wildlife, CITES, State Dept. of Land and Natural Resources.
[  ] Recombinant DNA, Genetically Modified Organisms, or recombinant field trials, i.e. USDA and HDOA.
[  ] Animals or Human Subjects uses must have authorization from the Institutional Animal Care and Use Committee (IACUC) for animals and the Committee on Human Subjects (CHS) for humans.

MEDICAL SURVELILLANCE

[  ] Has a biological commodity inventory been done and a copy sent to UH Biosafety Program.
[  ] If the use of human blood, blood products, cell lines, have all personnel involved in the project either received the Hepatitis B vaccination or been offered the vaccinations and completed a Hepatitis B immunization consent or declination form.
[  ] If human pathogens for which vaccines are available will be used, have all personnel involved in the project either received the appropriate vaccination(s), or been offered the vaccination(s) and completed a declination of Immunization form.

TRAINING AND EQUIPMENT CERTIFICATION

[  ] Have all personnel involved in the project reviewed the laboratory's Safety manual.
[  ] If human blood or blood products will be used, have all personnel involved in the project had HiOSH Biological Agents and Bloodborne Pathogen Training within the last year.
[  ] Have all personnel involved in the project been trained in Biosafety practices specific to the project.
[  ] Have all personnel involved in the project who will be shipping or receiving biological commodities been certified and trained by taking a Shipping and Receiving of Biological Commodities class.
[  ] Has the laboratory's biological safety cabinet, laminar flow clean bench,and autoclave been certified within the last year?
[  ] Are methods in place for proper disposal of biohazardous materials and are all personnel trained in proper biological waste's management practices, include sharp's disposal.
[  ] Have all personnel involved in the project been trained in biological spill clean-up and emergency procedures relevant to the project; including disinfecting, labeling, abatement, and reading and understanding of MSDS's.
