| The departmental brochure and/or the
department chair have provided you with a detailed information
about the importance of planning the Educational Technology
Master's degree program at the University of Hawaii.
Please, consider carefully the two time-sequence plans
and the explanation provided with each. The department
requires that you indicate which one of the two plans
(3- or 2-year) you think you will most likely to pursue
if admitted to the program. The final decision as to
which plan you will eventually follow will need to be
made at your first registration. Both plans assume that
any pre-program deficiencies had been removed prior
to the start of the program itself. The required courses
already inserted in each schedule are set and may not
be changed. The 2-year plan is rather ambitious,
and it is imperative that you take into consideration
all your personal commitments before you decide.

9 credit hours are considered a full-time load,
and students are discouraged to enroll in aditional
coursess. Exceptions may be considered in consultation
with the advisor, provided the student's performance
has been excellent. |
I am selecting the above option to undertake
the program of study in Educational Technology. I understand
the departmental rationale for prescribing the sequencing
of courses, and do acknowledge that the final decision
will have to be made at the time of my first registration
for the next Fall semester of classes. If ETEC 414 and
442 had not been enrolled in as yet, I shall remove
the identified pre-program deficiencies prior to my
beginning of the program, or during the first semester
at the latest. Submitting this form indicates your full
understanding of the program requirements and the criteria
and rules which govern the admissions to and operation
of the program.
