This form should be completed by all persons participating in GAW events. Please attach relevant programs, press clips, press releases, photographs, etc. Data is needed for a report to National Geographic Society. All completed report forms (postmarked by December 6, 2000) will be entered in a drawing for David L. Eyre's book, By Wind, By Wave: An Introduction to Hawaii's Natural History.
