The Committee on Human Studies may approve research for a maximum of
one year. Any research project which exceeds this time period
will be subject to ongoing or continuing review to renew its approval.
For most continuing research a completed Status Report Form (pdf|doc), plus a copy of the
current approved consent / assent forms (if applicable) is required
prior to the expiration date. In addition, a status report form should
be submitted for any change requiring review during the current
approval period.
Generally, the same submission requirements for the original
application, apply to continuing review. For a protocol which
requires full Committee review, 1 original and 13 copies of all
documents should be submitted .
Federal guidelines allow for some research under continuing review to
qualify for expedited review. Please check the categories eligible for
expedited review if you believe your application is eligible. If
your application is eligible for expedited review, 1 original Status
Report Form (pdf|doc) and 1 copy, plus two
copies of the current consent / assent forms (if applicable) is
required.
Determine which form, supporting documents, and number of copies are
required (see "Overview" above) for your application.
Drop-off applications at 1960 East-West Road, Biomedical Building,
B-104, University
of Hawaii Manoa, Honolulu, HI 96822.
For expedited review, allow about seven days for review if no
additional materials or revisions are needed. Some proposals make take
longer depending on the topic and proposed research activities.
Notes:
Incomplete applications will not be reviewed.
Renewal applications which require Full Board
Review should be submitted with one to two months leeway to avoid
periods of non-compliance, depending on the approval expiration date
and the Committee meeting schedule.
