how to apply application instructions


The following applies to Biomedical and Social Science / Behavioral research applications.

Please read before completing any forms or supporting documents:
  • Please fill out and print the application. (exception: handwritten Declaration of Exemption Coversheet is acceptable)
  • Make sure that all questions are answered. If a question is not applicable to your project, state "not applicable" or N/A.
  • Make sure that all of the required supporting documents are included with your application.
  • If you have additional information, such as explanations which may help clarify matters in your application, please attached a separate sheet or a memo.
  • Incomplete or illegible applications may be returned to you without processing.
  • Remember, the Committee is not as familiar with your project as you are. Providing sufficient detail and explanation in your application will help to avoid delay in the review and approval of your project. If you have any questions or require assistance, call the Executive Secretary / Compliance Officer (Bill Dendle 808.956.5007). The CHS staff will be happy to assist you in this process.
Form(s), supporting documents and number of copies required:
  • For new applications, not exempt, that will go for full Committee review, one signed original and 13 copies of the Application for New Approval Form (pdf|doc) and all supporting documents are needed, plus 3 copies of the full protocol/investigator’s brochure, if applicable.
  • For renewals with or without changes that must go before the Committee, one original signed Status Report Form (pdf|doc), and the current approved consent documents, plus 13 copies (no protocols or brochures unless part of the requested review) are required.
  • For renewals or minor changes within a period that qualify for expedited review, (please check to see what research qualifies for expedited review) an original signed Status Report Form (pdf|doc), and current consent documents, plus 1 copy of each are needed.
  • For exempt applications, 1 copy of the Declaration of Exemption Form (pdf|doc), a brief description of the proposed project and copies of consent documents, survey instruments, or questionnaire, if applicable, plus one copy of each is required. (2 complete sets)

  1. Determine which form(s) and/or supporting documents are required for your research. Please consult the sections on Exempt Review, Expedited Review, Continuing Research Review, and Full Board Review to guide you in making that determination.

  3. Allow the appropriate time for review (review timeline).
