Disability Access Services

Commonly used services include:

MP3 audio versions of some of these sections are available.  To listen to the file online, you will need to have an audio plug-in, such as QuickTime, installed on your browser.  If you wish to download the file for listening on your computer or portable MP3 player, please right-click the link and choose "save as" from the resulting menu, then save to your computer.

 

Other services include (but are not limited to): Scribe Services; Academic Adjustments (e.g. Foreign Language Substitution); Intra-campus Van Transportation; Furniture Placement; Classroom Relocation; American Sign Language Interpretation; Real Time Captioning Services; Assistive Technology; Bookstore Services; Library Assistance; Campus Advocacy; Physical Assistance; Mobility Assistance.

Eligibility for the above-mentioned services are determined by the KOKUA Program's Director upon review of professional disability documentation provided by the student and student interview.  Because needs change from semester to semester, current students are asked to meet with their KOKUA Counselor prior to start of the semester/summer session to discuss disability access services for the upcoming term.  We will only contact faculty on your behalf with your permission.  

 


EXAM ACCOMMODATIONS (MP3 audio version of Exam Accommodations)

Exam accommodations may include but are not limited to:
(The following are pre-determined by KOKUA staff based on disability documentation)

  • Extended time
  • Distraction reduced space
  • Assistive Technology (Jaws, Kurzweil, CCTV, etc.)
  • Live Reading
  • Scribe services
  • Alternate Media (Braille, E-text, Enlarged Copy, etc.)
  • Computer use (Word processing)
  • Answer sheet completion (Scantron)
You must submit a request to KOKUA at least one week in advance of your exam.  With the large volume of exams that we are arranging, we may not be able to accommodate late requests.  You can submit your exam request(s) one of three ways:

1.  Come into the office to schedule your exam in-person with our reception desk staff.

2.  Call KOKUA at 956-7612 or 956-7511 (voice/text).

3.  Email KOKUA at kokua@hawaii.edu.

Please notify your Professor of your intention to take his/her exam at KOKUA. Plan to start your exam at the same time as your classmates.  If not, any change must be approved by your Professor.

When making your request please provide us with the following information:

  1. Your name (e.g. Audrey Hepburn)
  2. Your contact information: (956-7612)
  3. Day, date, & time of exam: (Friday, February 17, 10:30 AM)
  4. Course (PACE 447)     
  5. Length of exam (e.g. 50 min., 75 min.) 
  6. Exam conditions (e.g. closed book, closed notes)
  7. Professor's name (e.g. Dr. Doolittle)
  8. Professor's contact information: (956-7511, iloveanimals@hawaii.edu)
  9. Accommodations requested (e.g. live reading, copy enlargement, scribe, JAWS, etc.)

Policies for exams at KOKUA:

•  KOKUA full-time staff will assign you to a testing area.

•  The following items are not allowed in the testing area:

v      Notes/books not permitted by the professor/instructor;

v      Any communication devices such as cell phones, PDA's, MP3 players, pagers, etc.;

v      Backpacks, bookbags and purses.

v Food or drink items.

•  Bags &/or valuables, as well as all communication devices (which should be turned off), can be placed in KOKUA’s locked front closet.

•  Faculty instructions/exam conditions will be reviewed with you by a full-time KOKUA staff member before the exam begins.  You will be asked to sign a document (green sheet) acknowledging that you understand the stated conditions.  You are responsible for abiding by these conditions.  KOKUA is responsible for enforcing these conditions.

•  You are not permitted to leave your testing area once you have begun your exam (exception is for bathroom use within the KOKUA office only).

•  You are responsible for your personal exam materials (i.e. pen, pencil, calculator).  If you forget personal exam materials and you leave to retrieve them prior to the exam’s start, when you return you will only be given the remainder of your allotted time.

•  If you are unclear about the exam instructions or conditions, or have any questions immediately stop the exam and seek assistance from a KOKUA staff member. The staff member will attempt to contact your Professor or allow you to write a note to your Professor explaining the problem or question which will then be returned with the exam.

•  All scratch paper will be provided by KOKUA staff and returned to your Professor with the exam. 

•  A KOKUA staff member may come into the exam room at any time to monitor exam conditions.

•  If testing irregularities are observed or if there is suspected evidence of cheating, the exam will be stopped, all materials will be confiscated and will be turned into the appropriate faculty member & the Office of the Vice-Chancellor with an incident report. As a result you may be charged with academic misconduct.

Cancellation, Lateness, Illness and Rescheduling Policies:

•  Students must arrive on time to take their scheduled exams at KOKUA. 

•  If you are unable to take your exam due to an illness or an emergency, contact your Professor immediately.  KOKUA is not able to make decisions about rescheduling exams.

 •  If you arrive late for your scheduled exam, the amount of time you are late will be deducted from your total exam time. 

•  If you decide not to take your exam at KOKUA, please notify our office as soon as possible so that we can utilize the exam space for other students and return your exam materials to your Professor.

•  If you do not show up for a scheduled exam, KOKUA will return the unproctored exam materials to your Professor.

•  It is your responsibility to initiate the exam request process to reschedule an exam.

FACULTY LIAISON

In faculty liaison service, KOKUA contacts a student's instructor(s) (with the student's permission) in order to facilitate accommodations such as exam proctoring or note-taking.

ALTERNATE MEDIA SERVICES (MP3 Audio version of Alternate Media Services)

Eligibility for this service is determined by the student’s respective KOKUA Counselor upon review of professional disability documentation provided by the student & the student interview.

Students must request alternate format services each semester, as needed.  Students initiate this process by submitting an "Alternate Format Request" form to KOKUA indicating the course materials and requested format needed (one form per course).

Students are asked to submit the "Alternate Format Request" form no later than the 1st week of instruction.  When at all possible, requests should be submitted well in advance of the semester’s start in order to receive media in a timely manner.  When learning about additional course material, please submit additional "Alternate Format Request" forms.  Please note that production time will be affected by late requests.   

Students are also asked to submit a copy of their course syllabi to KOKUA within the 1st week of instruction.  Inform KOKUA immediately of syllabi changes that will affect your alternate format request.  Also, if students drop a class and/or no longer need material that they have requested, they will inform KOKUA immediately.

Once a request is made, preference will be given to the format specified by the student.  However, KOKUA professional staff reserve the right to recommend specific media format in certain situations.  Instructional material will be provided in a single alternate format.

In compliance with federal copyright laws, student: (1) must own a physical copy of the course material and be able to provide a copy of the receipt; and (2) must agree that the use of alternate format materials provided by KOKUA is solely for his/her own educational purposes, and will not be duplicated/copied for use by others.

KOKUA will make every effort to complete the request in a timely manner and will maintain email communication with you about alternate format status.

Students must return all RFB&D materials and materials borrowed from other institutions to KOKUA at the end of the semester.

NOTETAKING (MP3 Audio version of Notetaking Policy)

Eligibility for this service is determined by the student’s respective KOKUA Counselor upon review of the professional disability documentation provided by the student and student interview.

Students must request notetaking each semester and specify need by course. 

Students who are requesting notetaking are responsible for providing paper.  Students must bring to KOKUA, (1) ream of folder paper and (1) ream of copy paper before the start of classes or as soon as notetaking services have been requested and approved.

Students must come to KOKUA to have their picture taken or provide a photo ID to be copied.  Photos will be shown to the notetakers so that they may identify the student in class.

Students must be present in class in order to receive notes.  If students are absent for two consecutive class periods and do not notify KOKUA professional staff in advance, notetaking will be suspended until the student speaks with his/her KOKUA counselor to reinstate notetaking.  Exceptions will be made for disability-related absences on a case-by-case basis.

Students must notify KOKUA immediately of any change in their schedule that affects notetaking (e.g. adding or dropping courses, professor changes, room changes, etc.) 

Prior to the first week of classes, students should check with their KOKUA Counselor about the status of their notetaking. 

Notetaking requests made after the semester has started will be honored provided that the students making the request understand that notetaking may not commence immediately but as soon as appropriate notetakers can be found and hired.
