Disability documentation can be submitted to KOKUA through mail, email, or fax.
Queen Liliʻuokalani Center for Student Services
2600 Campus Road, Room 013
Honolulu, Hawaii 96822
Documentation sent to KOKUA is reviewed by KOKUA professional staff and assists in determining appropriate accommodations. Documentation is kept in confidential files at the KOKUA Program office. Documentation will vary depending on the classification of disability. If documentation is determined to be insufficient, KOKUA may request additional documentation. Disability documentation written in a language other than English must be translated and notarized at the expense of the student.
Please feel free to view one of our documentation templates:
- Psychiatric Disability Verification Form (Doc)
- Mobility and/or Health-Related Disability Verification Form (Doc)
We ask that students retain their own copy of their disability documentation for future use as KOKUA will not indefinitely house this material and is unable to provide copies of documentation.
