
FORMS FOR STUDENTS
To receive credit for the externship, all forms must be submitted correctly and timely. All forms are to be turned in to the Externship Mailtray located among the faculty mailtrays.
1. Conflict of Interest Disclosure Form - submit this to the mentor before the mentor signs the Externship Memorandum of Agreement.
2. Externship Memorandum of Agreement - to remain enrolled in the externship program, this form must be completed BY THE MENTOR and submitted to the Externship Director no later than the end of the first full week of class. It is up to the student to ensure this form is completed, signed and returned to the Externship Director. The mentor should contact the Externship Director with any questions.
3. Time Sheets - each student enrolled in a part-time externship must submit weekly time sheets that must be initialed by the mentor indicating his/her review. See Section V for a sample.
a. hours: a student must work eight hours per week for fourteen weeks for atotal of 112 hours. It is not necessary to work the week of Thanksgiving or Spring break. If the student elects to work that week, the externship may be ended one week early.
b. lost hours: the student is responsible for making up any hours lost due to holidays, illness, etc. The student may not work extra hours one week and have those hours count for a later week or begin the externship early and receive credit for that time without the prior approval of the Mentor and the Externship Director.
4. Student's Critique of Externship Experience - must be completed and submitted to the Externship Director by the last week of class.

FORMS FOR MENTORS
It is the obligation of the student to deliver the forms to the mentor and assure they are completed and returned on a timely basis. If there is any difficulty in meeting this requirement, contact the Externship Director. All forms should be sent to the Externship Director. Externship Memorandum of Agreement - to be completed, signed and dated by the mentor and student. This must be delivered to the Externship Director NO LATER THAN the end of the first full week of class.
1. Mid-Term Evaluation Form - to be completed and signed by the mentor and delivered to the Externship Director by the seventh week of class.
2. Extern Appraisal Form - to be completed and signed by the mentor and delivered to the Externship Director by the last week of class.

MEETINGS, E-MAIL
1. Class meeting - there will be one class meeting during the first full week of class for all students registered for an externship. Check the bulletin board or your e-mail for exact date, time and location of the meeting. The Externship Director will review the procedural requirements for satisfactory completion of an externship.
2. Required individual meetings - during the course of the externship, each student must arrange for an individual meeting with the Externship Director. A sign-up sheet will be distributed at the first class meeting and will also be available at the Externship Director's office. At these meetings, the student's externship will be evaluated and the meetings should last between 20 to 30 minutes.
3. Office hours - the Externship Director is available for additional individual meetings. Please contact the Externship Director to schedule an appointment.
4. E-mail - communication will be via e-mail when possible. If your e-mail address changes, please advise the Externship Director immediately.
WRITTEN ASSIGNMENTS
All written assignments must be turned in on a timely basis to the Externship Director.
1. Journal - a BI-WEEKLY journal must be kept by the student and should reflect what the student has learned substantively and observed, including courtroom and office decorum, ethical behavior and professionalism. It should not simply be a more elaborate time sheet but rather should detail the student's reflections about the externship experience. A sample journal is provided in Section V.
Dates due: journal reports must be E-mailed to the externship director every two weeks.
2. Writing Samples - each student must submit TWO writing samples undertaken during the course of the externship. These may take a variety of forms such as drafts of letters, legal memos, position papers, pleadings, testimony, reports, manuals, etc. Confidential information, like the name of a client, may be redacted.

FINAL GRADE
The final grade will be based upon the student's performance in the externship, as evaluated by the mentor and the Externship Director.
