Checklist for SDC Award Applicant
- Award application form
- Original receipt, cancelled check OR copy of bank/credit card statement and corresponding check facsimile/credit card slip (The receipts are due after travel is completed.)
- Substantiation of event applied for. Examples of items to submit:
- Flyer/advertisement
- Copy of event registration form
- Copy of events confirmation
- Training Request Form (UH Form 410) [if applicable]
For OFF-ISLAND events, the applicant MUST submit copies of the following:
- Travel Request Form
- Travel Completion Form
