Course Proposal Procedures

  1. Be sure to use current versions of UHM-1 and UHM-2 forms and follow the current UHM guidelines . Additional information on the curriculum planning and approval process may be found on the OVCAA website (http://manoa.hawaii.edu/ovcaa/planning_approval/).
    Required: Justification for New Courses (UHM-1, box 19)
    Required: Justification for Course Modifications (UHM-2, box 9)
    Faculty/Departments should work with their PCC representative to make sure the paperwork is filled out properly before submitting curriculum forms to Carlo Ulep.
  2. Each proposal should be submitted as a single pdf file including both the UHM form and any additional documentation. Save the UHM pdf using Adobe Acrobat (not Reader), so that changes requested by the PCC can be readily incorporated. The syllabus must be attached to all UHM-1 and UHM-2 forms, Note: Learning outcomes need to be included.
  3. Proposals should be submitted, as email attachments, to Carlo Ulep (carlou@hawaii.edu) directly from the Department Chair. This is in order to ensure that the chair is aware of the proposal and will approve it.
  4. File names should be of the form “BIOL999V1.pdf”. The version number (e.g. V1) should be modified if changes and revisions are requested by the PCC.
  5. No signatures are required on the electronic submission. Once a UHM form has been approved, Carlo will work with the department chairs on getting signatures.
  6. Please enter “Natural Sciences…William Ditto” in the “Approved By/1st College/School…Dean” fields.
  7. Please enter the printed names of all other required signers in the appropriate fields. Do not enter a date on the signature line.

Changes to Degree Programs

A proposal to modify existing degree requirements should be in the form of a memo to the VCAA via the Associate Dean of the College. It is helpful if the memo includes a copy of the existing catalog entry marked up to include the proposed change(s). All major modifications in graduate programs require advance approval of the Graduate Council. Minor program modifications, such as changing or substituting a course, may be approved by the Graduate Division. Please refer to the GRAD Division website for specific information on the modification of graduate programs
