Reservation Form Instructions
Please follow the instructions below carefully.
1. COMPLETELY fill out the online reservation form. An incomplete form will delay the process of your reservation.
2. PRINT out TWO (2) copies of the reservation form. One will be sent to the NSP Office, and the other is for your records.
3. SIGN and DATE the "Orientation Agreement" section on both copies.
4. FULL PAYMENT is required to begin the reservation process. We accept credit card (MasterCard or Visa), check or money order payable to "University of Hawaii"
5. Send one copy of the reservation form and the FULL PAYMENT using one of the following methods:

- If you are paying by credit card, you may FAX your reservation form to:
Your reservation form AND full payment must be received before your reservation can be processed.
You will receive an EMAIL CONFIRMATION at your UH email address after we have received and processed your reservation form and payment. If you do not receive a reservation confirmation email within 2-3 weeks after mailing your reservation form, please email us at nso@hawaii.edu.
