All Forms 1 generated from this web page no longer have the Document Number pre-printed on each form. Each 6-digit Document Number is generated based on the following format:

digits 1 and 2 - Requestor Month (box 9)
digits 3 and 4 - Requestor Day (box 9)
digits 5 and 6 - Leave Code (box 3)

Note:

All Form Fillable versions automatically print the Document Number from the information submitted in boxes 3 and 9.
The Document Number must be unique for all Forms 1 submitted on the same day with the same leave code. Therefore, multiple requests must not have the same Requestor Date (box 9).

Two-Part Form Fillable
	This version utilizes Adobe/Acrobat Version 3 or higher form filling capability that allows the leave information to be entered via the computer. Two copies of the same information are printed on one sheet of paper. This version may be used in cases where a duplicate copy is necessary.

Three-Part Form Fillable
	This version utilizes Adobe/Acrobat Version 3 or higher form filling capability that allows the leave information to be entered via the computer. Three copies of the same information are printed on one sheet of paper. This version may be used in cases where triplicate copies are necessary.

Blank Non Form Fillable (2 or 3 copies per page)

This version utilizes Adobe/Acrobat to simply print out copies of the Form 1 to be manually completed. Any version of Adobe/Acrobat will allow you to print this document. The printed sheet may be cut to provide blank copies of the Form 1.
The Document Number on the Form 1 is not preprinted and must be manually entered by the employee. You may use the format indicated below to derive a unique document number.

Document Number Format:

digits 1 and 2 - Requestor Month (box 9)
digits 3 and 4 - Requestor Day (box 9)
digits 5 and 6 - Leave Code (box 3)

Example: The Requestor Date (box 9) is 09/15/98 (month=09 and day =15) and the Leave Code is 02 (box 3). Using the above format, the document number that must be entered in box 0 is 091502.
Note:

The Document Number must be unique for all Forms 1 submitted on the same day with the same leave code. Therefore, multiple requests must not have the same Requestor Date (box 9).
The format above using the requestor month, day and leave code is NOT mandatory but may be used to derive a unique document number. Any other algorithm may be used (eg., sequential starting from 000001, etc.) the only criteria being that the document number must be unique for an employee for the calendar year.

Hints when using the Macintosh for the Leave Forms:

If you receive a message "Some text in the font and character 'Arial 189' could not be displayed or printed correctly. The font could not be reencoded," you can click 'ok' and ignore this warning.

Printing the form

Check to see that you have selected the Paper size as US Letter (not US Letter Small). For example, if using a Laserwriter printer driver (8.3.4 and above), under File -> Page Setup -> select US Letter for paper size.

Use the 'Shrink To Fit' option when printing these documents from Adobe Acrobat.

Some printer drivers will have an option box on the first window stating 'Shrink to Fit'. Just select it by clicking an x in the box.
If using a Laserwriter printer driver (8.3.4 and above), under File ->Print -> click the arrowhead next to the box that says General. Select Acrobat Reader 3.01 (assuming that youre using version 3.01). Click an 'x' in the box so that 'Shrink To Fit' is selected.
The HP laserjet driver hasn't proven to be reliable when printing the form. Please use the laserwriter 8 driver instead.
