Lost or Stolen Cards / Canceling a Card

  Lost or Stolen Cards

(1)   Immediately call First Hawaiian Bank's 24-hour, 7 day a week Customer Service Center at 847-4444 (toll-free number: 888-844-4444). Identify yourself as a UH Purchasing Cardholder. The missing card will be cancelled and another one issued by First Hawaiian Bank. It is essential that you notify First Hawaiian Bank immediately because there is no limit to the University's liability for charges made on your PCard until it is reported lost or stolen.

(2)   Your Department will be responsible for all charges made on your PCard until you notify First Hawaiian Bank.

(3)   At the first opportunity during business hours, inform your Fiscal Office of your lost/stolen card and notify First Hawaiian Bank in writing.

(4)   Your Fiscal Officer should contact the Purchasing Card Administrator, who will follow up with FHB if necessary.

Canceling a Card

Return your Purhcasing Card to your Fiscal Office upon:
  • Termination of employment
  • Transferring departments within the University
  • Change in employment that no longer includes the purchasing function
  • Upon request from the Purchasing Card Administrator, Approving Official or Department Liasion
Cancel your card via one of the following methods:
  • Call 956-8687
  • Send email to Gwen Won or Joyce Teraoka
  • Mail a letter to UH Purchasing Card Administrator, OPRPRM, 1400 Lower Campus Road, Room 15, Honolulu, HI 96822.
In addition, cut the Purchasing Card in half and send it to the UH Purchasing Card Administrator at the above address.
