WAIVERS AND MEDICAL CONSENT FORMS

Whenever potentially or inherently hazardous and dangerous conditions exist in any University-sponsored activity, all participants should be required to complete an Assumption of Risk and Release form prior to participation. Such activities include but are not limited to:

• Field trips or other off-campus activities
• Laboratory
• Apprenticeships
• Practicums
• Intramural athletics, etc.

In some cases, a Medical Consent Form should also be required.
