The medical record is necessary to document the care and services provided to you and to insure a continuity of care. ACCESSING YOUR MEDICAL RECORDS All medical records are treated in a strictly confidential manner. Only authorized UHSM/LCC personnel have access to your medical records. Your written authorization is necessary to release any information, except when required by law or a court order. You may request a copy of your medical records by submitting the Release of Information Authorization Form to our office.
