REGISTRATION
Registration for the PCCHA 2007 conference is now available. Registration includes access to all sessions and the exhibitor hall, continental breakfast Monday through Wednesday, lunch Monday and Tuesday, dinner Sunday and a special off-site sunset reception at the Waikiki Aquarium on Tuesday. Additional meal tickets may be purchased for the dinner Sunday, lunch Monday and reception Tuesday. All other meals are reserved for registrants and exhibitors.

A special early bird registration rate of $395 is available for those who register before September 15th. Anyone planning on applying for CE units must pay an additional fee of $25.

Payment may be made by check (payable to PCCHA), VISA or MasterCard. To register, click on the *.pdf document to the right.

Important Information for Users of Acrobat Reader: Reader does not allow you to save changes to the form. If you are using Reader you should be sure to print out a copy of the completed form for your files.

There are three options for sending us your registration:
  • By Email: If you are using Outlook or Eudora you should be able to email the data in the form directly to us using the email button at the top of the page. Other email systems may not work with the *.pdf file. If you are using Adobe Acrobat Standard or Professional you can save the changes and send it to our email as an attachment. You may only send the form via email if you are using a credit card.

All hotel and travel arrangements are the responsibility of the registrants. The hotel reservation form is available on the website and must be submitted directly to the Ala Moana Hotel.

Cancellation policy: In the event you must cancel your registration written notification must be made. Refunds will be available as follows:

  • 100% of the registration before September 15th
  • 75% of the registration before September 30th
  • 50% of the registration before October 15th
  • 25% of the registration before October 30th
