WOMEN'S HEALTH - FINANCIAL HARDSHIP From time to time the UHSM Women's Health Clinic has federal funding available for assistance with contraceptive supplies. In order to claim financial hardship to qualify for discounted contraceptive supplies a Fee Determination form must be completed. The WHC provider will determine the discounted fee.
