On this page there is also some general information and forms typically used by the UH Student Caucus. Note the schedule that has been set up for all times and locations of when and where the UH Student Caucus will meet for the 2008-2009 school year.

3) Travel Request Form

Be sure to keep on top of deadlines regarding your Travel Request Form submission. Make sure to use this form when you send it in to Beverly Shigemura at the Office of VP for Student Affairs. Here's her email bshigemu@hawaii.edu and the number to fax her at is (808) 956-9219. Please remember to fill out your travel request forms and submit it to her in an attachment, don't send the information written on an email. (download the travel request form) NEW FORMAT!
