The CARE Act goes into effect July 1st which requires Hawaii’s acute hospitals to:
- Provide the patient with the opportunity to designate a caregiver (family, friend, neighbor, etc.) on their medical record;
- Notify the caregiver prior to the patient’s discharge or transfer to another facility;
- Offer the caregiver instructions on the patient’s post hospital care.
The CARE Act helps family caregivers when their loved ones go into the hospital and as they transition home.
