FALL 2020 SEMESTER
NEW FISCAL YEAR FORMS
We are gearing up for a new school year and we require ALL families to turn in the following documents by July 31, 2020.
New families who have recently submitted these forms for their Summer or Fall enrollment must complete these forms again as they must be dated after July 1, 2020.
2020-2021 New Fiscal Year Forms
- 2020-2021 CACFP Meal Benefit Form & Instructions: This Federally required form is specific to this fiscal year. All families must complete this form, including new families who just completed it for the Summer or Fall start. If you do not wish to disclose your income, you may indicate “Above Scale” as your income, and you will fall into our Step 5 for tuition rates.
- 2020-2021 Identification & Contact Card:Please complete a new emergency card. You must indicate at least 2 adults (other than the child’s parents) as authorized pick up/emergency contacts. Emergency contacts must be on the island of O‘ahu.
- 2020-2021 Meal Enrollment Statement: Please complete this required form with your child’s schedule for the Fall semester. If you indicate your pick up time after 2:00 pm on Fridays, we will automatically add Friday care to your child’s schedule.

Fall Child Care Service Agreements: Fall Service Agreements will be emailed during the first week of August. Agreements must be completed and returned in order to secure your childcare space. If you are a student, you are required to indicate the number of credits you are taking for the Fall and your projected graduation semester and year. The Service Agreement is our contract with you, it is NOT A BILL. Childcare subsides and work credit hours will be deducted from this amount. Tuition rates for the new school year can be found here.
The following documents are also being accepted:
- If you are receiving any outside childcare subsidies (Preschool Open Doors, Child Care Connections, PKS, First-to-work) please give us a copy of your Notice of Payment form or award letter.
- If your child has a chronic illness or allergy (i.e. bee stings, nut allergy, etc.) please submit the following medical forms. These need to be submitted annually. Forms are available at Documents for Currently Enrolled Families.
- If your child has a chronic illness or allergy:
- DHS 908 Early Childhood Pre-K Health Record Supplement (completed by Doctor)
- Chronic Illness/Allergy Health Record (completed by Doctor)
- Chronic Illness/Allergies Permission to Administer Medication
- If your child has an EPI pen:
- UHMCC Food Allergy & Anaphylaxis Emergency Action Plan Form (completed by Doctor)
- If your child has a milk allergy or milk preference:
- DHS 908 Early Childhood Pre-K Health Record Supplement (completed by Doctor)
- Policy for Milk Intolerance
All families are assessed an Annual Comprehensive Fee of $125 at the beginning of the Fall Semester. This annual fee is in addition to your child’s tuition. If your child began their enrollment with us this past Summer or will just begin this Fall Semester, your Annual Comprehensive Fee will be covered by your deposit. If you are receiving Preschool Open Doors, they will pay for your comprehensive fee. If you are receiving Child Care Connections, please check your notice of payment from them to confirm if they will be paying for your comprehensive fee. If you are receiving PKS, please pay the comprehensive fee as PKS does not pay for this fee.
