Medical Home Model Practice Module
The Medical Home Model Practice is an attempt to go beyond the traditional emphasis on “continuity: in a resident’s ambulatory care practice to begin stressing other non-cognitive skills needed for effective community-based practice.  These “other skills” are being increasingly recognized by national organizations (such as the Accreditation Council on Graduate Medical Education, the Future of Pediatric Education II, and the American Academy of Pediatrics) as skills that need as much emphasis during residency training as the concept of “continuity”.  To accomplish this, you will be trained in and incorporate the philosophy of the Medical Home (compassionate, culturally effective, family-centered, coordinated, comprehensive, accessible and continuous care) into your continuity care practices.  You will also be introduced to and utilize evidence-based practices and nationally recognized health supervision guidelines.

Goals for this module:
The goals of this module are for pediatric residents to understand and apply the Medical Home principles within continuity care practice.  Emphasis is on the ability to demonstrate continuity, accessibility, comprehensive and coordinated care that is delivered with family centered, culturally effective and compassionate care to actual patients in practice.
 

ACGME Competencies to be demonstrated upon completion of this module:

1.      Knowledge and Patient Care: Knowledge of Medical Home principles with delivery emphasis on family centered, compassion and culturally effective care as stated in the American Academy of Pediatrics Policy Statement 2002.

 

2.      Interpersonal Communication: Interpersonal and communication skills that result in effective information exchange and teaming with patients, the patients’ families and professional associates.

 

3.      Professionalism: Professional behavior in upholding ethical principles and maintaining cultural sensitivity to a diverse patient population.

4.      Systems Based Practice: Appropriate utilization of community-based resources in our local system of health care to optimize management and provide care that is optimal value.

5.      Practice Based Learning: Self-evaluation of personal patient care practice to improve individual patient care practices.

 

Objectives:  Upon completion of this module, pediatric residents will be able to demonstrate:
1.      Knowledge of the seven medical home principles
on a written exam.

2.     Family centered, culturally effective, and compassionate care to clinic patients.

3.      Professionalism through effectively communicating with others as a member or leader of the health care team.

4.      Appropriate utilization of health care resources, consultants, and services.

5.      Practice-based learning through self-evaluation and monitoring of personal Medical Home practice patterns.
