Application for University of Hawaii Scientific Diver Authorization

PLEASE READ BEFORE BEGINNING THE APPLICATION

  • The Scientific Diver Application is a Google Form.  It must be completed in one sitting. 
  • This form accepts responses from all email accounts, but if possible, please fill out this form using your @hawaii.edu individual email address.
  • There is no option to start this form, save it, and return at a later time. 
  • The application allows the upload of supporting documents, such as dive logs, c-cards, training verification letters, and/or (for non-employees) DAN Diving Accident Insurance. Before starting the application, please ensure that you have electronic copies of anything you wish to upload. Documents may be *XLS, *.DOC,  *.PDF or *.JPEG files.  The number of uploads is limited, so compiling multiple files (e.g., one file with multiple certifications) is advised. While these documents may be submitted at a later date, inclusion at this time will greatly expedite and simplify your application process. 
  • The UH Diving Control Board has mandated that COVID19 vaccination is now required to participate in UH diver training. If you have not previously done so, Please upload a copy of your current COVID19 vaccination card, to be included in your UH diver medical record.
  • Before starting the application, please ensure that you have scanned copies of supporting documents you wish to include, such as dive certifications, dive logs, and AAUS training verifications, and (for student non-employees) DAN Diving Accident Insurance. Documents may be uploaded as *XLS, *.DOC,  *.PDF or *.JPEG files, however the number of uploads is limited, so compiling multiple files (e.g., one image with multiple certifications included) is advised. While these documents may be submitted at a later date, inclusion at this time will greatly expedite and simplify your application process.

  • Due to recent pandemic-related delays in training, space in upcoming Scientific Diver Qualification Courses is expected to be very competitive.  Priority will be given to applicants based on employment and academic standing who demonstrate an immediate and essential need for qualification. If you are applying for this purpose, please upload a scanned letter from your advisor, explaining the essential and immediate nature of the need, and the irremediable impact to your UH position if you cannot be accommodated.
  • Once submitted, your application will be reviewed. After preliminary approval, it will be returned to you for your and your advisor’s final signatures.

Visiting Diver Non-Governmental Scientific Diver Application

  • This form is for employees of Federal agencies (NOAA, National Park Service, USGS, etc…) State agencies (Hawaii DLNR-DAR, etc…); Government colleges, universities and research/educational institutions.

  • Please fill out this form using your personal individual email address. If you have a @hawaii.edu email address, please use it.

To comply with State of Hawaii regulations on the use of volunteers, liability coverage for the UH, PI and Lead Diver, and provide worker safety coverage for the volunteer.  UH Principal Investigators who propose to use UH scientific divers as volunteers for their UH project should complete and submit this form with original signatures to the UHDSP for each participating diver.

NOTE: The UH Volunteer Form should be completed and on file with the project’s PI/Department before the divers initial participation in any UH activity.
