Annual UH Personal Information Survey
Adding a New Survey
A survey must be filled out for each existing Personal Information System within your department, program, or unit.
Getting Started
- Go to My Surveys
- Review My Contact Information and update if necessary
- Name of Survey (required)
Examples
- Name of system: Banner, PeopleSoft, Laulima
- Description of system: personnel files, job applications, school applications
- Campus/Organization (required)
- Name of Subunit (optional)
- Click Add Survey
Filling in the Survey
- Each question requires a response; if not applicable enter "n/a"
- For helpful hints, hover over the text box for further explanation or examples
- Click Save & Close to continue at a later date
- Click Submit Survey when complete
Annual Reporting
- Previously reported systems can be reviewed and updated annually without filling in a new survey.
- New systems added since the previous reporting year will require the addition of a new survey.
