Annual UH Personal Information Survey


Adding a New Survey

A survey must be filled out for each existing Personal Information System within your department, program, or unit.

Getting Started

  • Go to My Surveys
  • Review My Contact Information and update if necessary
  • Name of Survey (required)
    Examples
    • Name of system: Banner, PeopleSoft, Laulima
    • Description of system: personnel files, job applications, school applications
  • Campus/Organization (required)
  • Name of Subunit (optional)
  • Click Add Survey

Filling in the Survey

  • Each question requires a response; if not applicable enter "n/a"
  • For helpful hints, hover over the text box for further explanation or examples
  • Click Save & Close to continue at a later date
  • Click Submit Survey when complete

Annual Reporting

  • Previously reported systems can be reviewed and updated annually without filling in a new survey.
  • New systems added since the previous reporting year will require the addition of a new survey.
