Project title: Human Papilloma Virus immunization rates in adolescent females

Project description: I suspect that I have many adolescent females in my practice who have not been immunized with human papilloma virus (HPV) vaccine. It is more difficult to achieve high immunization rates in adolescents since they do not seek primary care on a regular basis as often as infants and children. I would like to improve my practice’s HPV immunization rate for adolescent females. The aim of this project is to increase the number of adolescent females age 11 -21 in my practice who are screened at every visit and who receive the HPV vaccine during the study period.
Measurement: Review charts of adolescent females. If the patient has completed at least two HPV vaccine doses, then score this as a “yes”. If not, then score this as a “no”.
This measure is determined by the PCL to be a two-category variable.
Performance and Improvement will be measured by percentages of the above.
Sample size: I will be required to review charts of patient encounters of 10 or more patients described in this project description and complete the measurement described above.

Review and summarize baseline data within 2 weeks
Project community leader (PCL) summarizes these reviews and summaries within 1 week
Participants submit performance improvement method proposals within 2 weeks
Participants review all proposals submitted by community, then choose their method(s) within 2 weeks
Participants implement their performance improvement method(s) into their practice within 4 weeks
Practice in operation with performance improvement method(s) in place for 8 weeks
Follow-up data collection within 4 weeks
Review and summarize follow-up data within 2 weeks
PCL summarizes reviews and summaries within 1 week
Participants review PCL summary and submit project evaluation within 2 weeks
