Step 10: Complete follow-up practice data collection and post on web site.

During this step, participants will receive this E-mail message to start step 10.

Date: August 20 [due date 9 + 1 day]

Dear [project participants]:

As of August 19 [due date 9] your performance improvement method(s) for the MYQIP project HPV Immunization Rates In Adolescent Females, should have been fully operational in your practice for at least 8 weeks. Do not change this. Continue to operate your practice with your performance improvement method(s) in place.

The next step is to collect follow-up data. This should be done in the same way that you collected your baseline data. Your sample could be different patients in your practice, or the same patients who were studied earlier, or a combination of the two. You must review 10 (or more) patient encounters or patient charts using the study measurement that was used during the baseline data collection period. When you complete your observations (minimum sample size is 10), you must enter your data on the MYQIP project web site. You will need the number of observations (n) and [the number of successes or positive results or the mean value of these observations, MYQIP selects the proper option depending on the variable type that was selected].
