Date: enrollment date
Dear Dr. Robert Poth: Thank you for participating in the MYQIP project HPV Immunization Rates In Adolescent Females. I have the privilege of being your project community leader (PCL). Together we will collect data, reflect on this data, make recommendations to improve our practice based on this data, then collect a follow-up set of data, and reflect on this data again to see if we have succeeded in improving our practice. Enrollment for this MYQIP project will close on February 26 [due date 1]. All participants must complete the QI basics course by March 11 [due date 2]. Completing the Quality Improvement (QI) Basics course is required prior to starting this project. You can enroll in the QI Basics course and complete it now or any time prior to the start of this MYQIP project. Go to the AAP Pedialink web site (www.pedialink.org) to register for the QI basics course. The group will be given an additional two weeks to complete QI Basics before the MYQIP project starts. When you complete the QI Basics course, this will be automatically entered on the MYQIP project web site [web site URL]. Project description: I suspect that I have many adolescent females in my practice who have not been immunized with human papilloma virus (HPV) vaccine. It is more difficult to achieve high immunization rates in adolescents since they do not seek primary care on a regular basis as often as infants and children. I would like to improve my practice’s HPV immunization rate for adolescent females. This project is justified by the following citation: AAP Committee on Infectious Diseases. Prevention of Human Papillomavirus Infection: Provisional Recommendations for Immunization of Girls and Women With Quadrivalent Human Papillomavirus Vaccine. PEDIATRICS 2007;120(3):666-668. To receive Maintenance of Certification Part 4 (MOC-4) credit from the American Board of Pediatrics (ABP), you will have to complete all aspects of this project according to the timeline below. If you do not comply with the timeline and the participation requirements, no credit will be given. The timeline for this MYQIP project is listed below. The minimum sample size for the baseline and follow-up periods will be 10 observations. These can be patients from your practice, and/or the practice of your group (includes your partners) to be HIPAA compliant. Sincerely yours, Loren Yamamoto, MD, FAAP Your Project Community Leader (PCL) [insert timeline here] |
