Dear [project participants]: As of April 22 [due date 4] you should have entered your review and reflection on the baseline data on the MYQIP project web site for the MYQIP project HPV Immunization Rates In Adolescent Females. The project community leader has reviewed and summarized all of these reviews and reflections. This summary is posted on the MYQIP project web site. The next step is for you to propose a method of improving the performance of your practice as it relates to this data. You must enter these proposed methods of improving performance on the MYQIP project web site by May 13 [due date 6]. You can review the proposed performance improvement methods posted by others in the group. By May 27 [due date 7], you must decide on one or two performance improvement methods that you will actually implement in your practice. You will have 4 weeks to implement these performance improvement methods into your practice and an additional 8 weeks for your practice to operate with these performance improvement methods in place. Following this period, you will repeat the data collection procedure that was done to collect the baseline data to see if the improvement methods have resulted in improvement compared to baseline. Each participant can choose different improvement methods since each practice is different. This will also permit all participants to see which performance improvement method worked better. At this point, the next step is for you to review the baseline data review and reflection summary posted by the project community leader and enter your proposed methods of improving the performance of your practice as it relates to this data, on the MYQIP project web site by May 13 [due date 6]. As your project community leader, I am obligated to confirm that all participants complete all program participation requirements according to the timeline in order to receive MOC-4 ABP credit.
