Dear [project participants]: As of June 24 [due date 8] you should have fully implemented the performance improvement method(s) into the operations of your practice for the MYQIP Project: HPV Immunization Rates In Adolescent Females. The next step is to continue to operate your practice with these performance improvement methods in place until the completion of this MYQIP project (and potentially beyond if you find that these methods improved your practice). With these performance improvement methods in place for at least eight weeks, success will result in an improvement in your practice. When this period of operations is completed on August 19 [due date 9], data collection (the same measurements that were done during the baseline period) will be repeated and compared with baseline to determine if improvement has occurred. You will receive weekly reminders to continue the operation of your practice with the performance improvement methods in place for this period. The day after August 19 [due date 9] you receive an e-mail notification, that it is time to begin the collection of follow-up data. As your project community leader, I am obligated to confirm that all participants complete all program participation requirements according to the timeline in order to receive MOC-4 ABP credit.
