This form should be filled out by every UH Community Member and/or Non-UH Volunteer if they are responsible to drive minors in their approved program. It is the CPA’s responsibility to retain the signed forms for each program.
Step 1. Fill out program name and name of driver.

Step 2. Fill out contact information in case there is an accident. Contact information should be the CPA for the program.

Step 3. Participant prints full name, dates, and signs.

Step 4. CPA to acknowledge that the participant has a valid driver’s license.
