What are the Leeward CC health clearance requirements?
Leeward CC health clearance requirements include:
- Tuberculosis clearance from the age of 16 or older
- Immunization clearance for Measles, Mumps, and Rubella (MMR), Chickenpox (Varicella), and Tetanus-Diphtheria-Pertussis (Tdap)
How can I meet the tuberculosis health clearance requirements?
You can meet the TB health clearance requirements by providing documents that show you have no risk of TB or are clear from tuberculosis. The following documents are accepted.
- TB risk assessment done using the State of Hawaii Risk Assessment Questionnaire (PDF)
- TB skin test and chest x-ray done in the US
- QuantiFERON (TB blood test) performed in the US and outside of the US
Where can I receive tuberculosis clearance?
- Your primary care physician, who must be a US licensed healthcare practitioner (M.D., D.O., A.P.R.N., or P.A.)
- The Leeward CC Student Health Center. We are located in AD-223. Our address is 96-045 Ala ‘Ike, Pearl City, HI 96782. We are open Monday through Friday from 8:30 am to 3:30 pm.
- Hawaii State Department of Health - Lanakila Health Center. Please call (808) 832-5731 for location and hours.
What are the immunization requirements?
Immunization requirements include proof of immunity from Measles, Mumps, and Rubella (MMR), Chickenpox (Varicella) and Tetanus-Diphtheria-Pertussis (Tdap).
- MMR
- Submit documents that show you had two doses of MMR. Your first dose must have been given on or after your first birthday and the second dose must have been given at least 4 weeks after the first dose.
- MMR requirements may be waived if you were born before 1957.
- Varicella
- Submit documents that show you had two doses of Varicella. Your first dose must have been given on or after your first birthday and the second dose must have been given at least 4 weeks after the first dose.
- Varicella requirements may be waived if you were born in the US before 1980.
- Tdap
- Submit documents that show you had one dose of Tdap on or after age 10.
Can I submit MMR titers instead of documentation of 2 MMRs?
No, although positive titers for Measles and for Rubella are accepted, the Hawaii DOH does not accept the Mumps titers. Therefore, documentation of 2 MMRs are required.
Where can I receive these immunizations if I am unable to locate my immunization records?
- Your primary care provider, Kaiser, Military clinic
- Any clinic, pharmacy, or urgent care that provides immunizations
- The Leeward CC Student Health Center. We are located in AD-223. Our address is 96-045 Ala ‘Ike, Pearl City, HI 96782. We are open Monday through Friday from 8:30 am to 3:30 pm.
I am currently not in the United States. Is it possible to meet the Leeward CC health clearance requirements before coming to begin school?
Yes! To meet TB requirements, you may submit any TB clearance done on or after the age of 16 as long as it was done in the US. TB risk assessments must have been done using the State of Hawaii Risk Assessment Questionnaire (PDF) . Or you may get the TB blood test called the QuantiFERON. This blood test may be done outside of the US. To meet immunization requirements, you may submit your childhood immunization record or be re-immunized where you are.
