Supplemental Financial Aid Forms
Students applying for financial aid at one of the University of Hawai‘i System campuses may need additional documentation to complete their aid application. The following forms are accepted by all campuses except UH West O‘ahu and UH Manoa. Check with the financial aid office at these institutions for campus specific forms.
