Understanding 360° Survey
What is the 360° Survey?
The 360° Survey is an anonymous feedback tool used in the E/M Performance Evaluation process to provide a well-rounded assessment of an employee’s leadership, communication, decision-making, and overall performance. It gathers insights from multiple perspectives to support fair and balanced evaluations.
Purpose: To supplement the supervisor’s evaluation with peer and subordinate feedback.
Survey Format: Multiple-choice ratings and open-ended responses covering key performance areas.
Anonymity: Your responses are confidential and will only be shared with supervisors as aggregated data. There is no identifiable connection between your login information and your assessment responses, as they are submitted and stored separately, ensuring anonymity.
The 360° Survey follows a structured process each year:
- Evaluator Selection: Employees update their 360° Evaluator list, and supervisors approve it.
- Survey Distribution: Approved evaluators receive an email invitation with a unique link to the survey.
- Survey Completion: Evaluators provide anonymous feedback by answering rating-based and open-ended questions.
- Data Compilation: Responses are aggregated and prepared for review.
- Supervisor Review: The E/M employee’s supervisor receives the 360° Survey results and incorporates them into the final evaluation.
Important: Individual survey responses are not shared—only summarized data is provided to supervisors.
Each year, all E/Ms eligible for the performance evaluation will receive a 360° Evaluator Excel Spreadsheet from their Department Coordinator containing a list of 360° Evaluators. Evaluators are selected from the following groups:
Peers/Constituents – Colleagues, governance/advisory group members, and partner organizations who work with the E/M employee.
Subordinates – Employees who directly report to the E/M employee.
Supervisors – The employee’s direct manager, who incorporates survey results into the final evaluation.
Who selects the evaluators?
- E/M employees update their 360° Evaluator list annually.
- Supervisors review and approve the final list before the survey is distributed.
The qualities assessed in the 360° Survey include the following (and are also performance criteria in E/M evaluations):
- Impact and Achievement
- Planning and Timeliness
- Resource Management
- Interpersonal Impact
- Communication
- Decision-making
- Leadership/Management
- Equal Opportunity and Civility
- Supervisors review aggregated survey results to gain insights into the employee’s performance.
- Feedback is used alongside the employee’s self-evaluation and the supervisor’s own observations to complete the final evaluation in Trakstar.
- Survey results help identify leadership strengths and areas for growth.
The 360° Survey is NOT a standalone performance rating. It serves as an informational tool to support fair and comprehensive evaluations.
Receive 360-degree Survey Participation Email
To begin the survey, click on the URL in the 360-degree E/M assessment email sent to you.
If You Are Selected to Evaluate Multiple E/Ms:
You are asked to complete an assessment of each of the executives in the drop down list. Please select one of the executives from the list to begin the process.
On the next screen, confirm name of E/M.

The 360-degree Survey Rating Scale
You are asked to rate the E/M’s qualities on a scale from 1 below expectations to 7 exceeds expectations. Your feedback is considered by the designated administrator for the annual evaluation and used for development purposes.
Not Applicable: Choose “NA” (Not Applicable) if you feel you are not able to complete that assessment.
Rating score of 2 or below: If a score of 2 or below is given, we would appreciate your comments on areas that should be addressed. Additional comments may be added in the “comments” box located at the end of the survey.

Finish Survey – “PAU!”
To complete the survey:
After clicking on the “PAU!” button at the end of the survey, your responses will be anonymously stored and you will be unable to return at a later time to change or delete your responses. DO NOT click “PAU” button for an incomplete E/M survey.
To leave the survey and return at a later date:
You may terminate the survey at any time and log back in at a later date to complete the survey. However if you exit before completing and submitting the survey, your responses will not be saved. Therefore, when you return to this survey you will need to start over at the beginning.
To Evaluate Another E/M
Click the Next button to continue to the next E/M survey or click the original URL from the email to select another E/M.
FAQs
The 360-degree surveys are optional, if you do not feel comfortable responding, please do not feel obligated to participate.
The comments may be summarized and shared with the executive and managerial employees who understand that it should be kept highly confidential.
No, the system was designed to ensure confidentiality therefore, there is no identifiable connection between your login information and your assessment responses.
The responses are submitted and stored separately from the login information and, therefore, it will not be possible to trace back to an evaluator’s response.
Once you click on the “PAU!” button at the end of this survey, you will be unable to return at a later time to change or delete your responses.
You are still eligible to participate in the 360-degree Survey, however, if you do not feel you have interacted sufficiently with any of the listed E/Ms or do not feel comfortable responding to the survey, please do not feel obligated to participate.
