Request Term-Specific Services

If you have completed an intake and discussed specific accommodations with your KOKUA Counselor, remember to request term-specific services by filling out a form related to the semester/term.

Disability conditions may be dynamic and course content also may vary. Please contact KOKUA well before the term begins, or as soon as the need for accommodation is known. Accommodations are not applied retroactively. KOKUA will not disclose your disability status or otherwise arrange for any term-specific services without your express permission/request. This includes but is not limited to services such as sending out faculty liaison letters or arranging for student housing accommodations, notetaking services, exam accommodations, alternate format production, etc.
